• First of all go to www.Foxyform.com.
• Then customize your form as per your requirements as there are many customization options are available.

• After customizations enter your email and click on "Create Formular".
• Now it will provide you a HTML code on the next page.
• Copy the code and log in to your Blogger Account.

• Then Go to Dashboard >> Pages >> New page >> Blank page.

• On this blank page enter your title eg. Contact us, and the go to HTML.

• In HTML section paste the code that you have copied from Foxyform.
